The Greater Johnstown Community YMCA (Y) offers a variety of Wellness and Chronic Disease Programs to members of all ages so that they might enjoy a more healthy and fulfilling life.

Community-Based Chronic Disease Programing

In communities across the nation, the Y is a leading voice on health and well-being. For over 15 years, the Y has driven innovation in prevention and control of chronic disease to improve quality of life, save lives, and reduce healthcare spending. With support from the Centers for Disease Control and Prevention, the Y delivers tangible results in scientific evidence-based chronic disease prevention and control. Across America, Ys are increasing opportunities and policies promoting physical activity, expanding health and wellness initiatives, and offering health care solutions and education outside the traditional healthcare model.

The Greater Johnstown Community YMCA is proud to offer a wide-variety of chronic disease prevention and control programs and meeting the expanding needs of our region. Taking a community-based approach to health care, our team is able to deliver medical care and education in a community setting at low or no cost as part of our mission to combat chronic diseases in our region.

Self-Monitoring Blood Pressure Program

High blood pressure is often referred to as “The Silent Killer” because there are typically no warning signs or symptoms. High blood pressure is a key, modifiable, risk factor for both heart disease and stroke, which are two of the leading causes of death in the United States.

The YMCA’s Self-Monitoring Blood Pressure (SMBP) program is designed to help adults with high blood pressure lower and manage their blood pressure through healthy diet and exercise. This is a four-month program that focuses on regular monitoring one’s blood pressure through proper techniques, one-on-one consultations with a Heart Healthy Ambassador (HHA) twice a month, support and group-based nutrition education delivered on a monthly basis.

Goals of the program: 

  • Measure and record blood pressure, at least twice a month, to learn triggers that impact blood pressure readings.
  • Learn how to properly take your blood pressure.
  • Meet with a HHA twice a month to review blood pressure, goals, and health.
  • Learn nutrition for achieving a healthy blood pressure through attending monthly nutrition seminars.

Who Qualifies?

  • Must be at l8+
  • Be diagnosed with high blood pressure or currently taking medication for high blood pressure
  • Not have experienced a recent (in the last 12 months) cardiac event 
  • Not have atrial fibrillation or other arrhythmias
  • Not be at risk for lymphedema
